Police Department Forms Center
Fillable forms can either be filled out on your computer or printed and filled out by hand. They will need to be printed, signed (if applicable), and submitted. Concealed Pistol License and Firearms Dealer License Applications must be submitted in person to the Port Angeles Police Records Department by the person applying. All other forms can either be placed in one of the drop boxes at each of the Port Angeles Police Department entrances or turned into the Port Angeles Police Records Department. An ADA Officer is also available to assist with the submission of these forms.
